Schedule an Appointment
To schedule an appointment:

If, for any reason, you cannot keep a scheduled appointment, or will be delayed, please let us know by calling.
Check In
When you arrive, someone at our front desk will confirm that the information we have on file is correct. If you are a new patient or need to complete updated paperwork, that will be completed at check in as well, please arrive 15 minutes prior to complete paperwork. You can also complete the paperwork online, prior to your appointment. To do so, click here.
If you have a copay, you are welcome to pay it prior to your appointment. We may also collect a copy of your driver's license and insurance card if it's needed.
Seeing your Provider
When seeing your provider, you may be asked to change into a gown, especially for a skin check appointment. If you have any specific areas of concern, please let your provider or their assistant know as your provider’s assistant will be in the room during your visit to help. Your provider will examine your skin and go over any treatment options.
If a biopsy is needed, it will be taken in your appointment. If the patient's concern requires surgical removal, a separate appointment is made.
Check Out
Once you have seen your provider, you can stop at the check out desk to make any follow up appointments, or pay a copay or existing balance. If going through insurance, we will submit your claim. If there's any portion not covered by insurance, you will receive a statement in the mail.
Additional Helpful Information
Insurance and Billing
We accept most traditional insurance plans. Please contact our office to verify acceptance of your insurance carrier, but it is always best to check with your insurance company to ensure we are in-network with your specific plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
Cancellation and No Show Policy
We understand things will come up which may require canceling an appointment. Please call us as soon as possible to let us know if you will have to miss an appointment so that we may fill the appointment slot. We, unfortunately, needed to incorporate a policy and fee due to a high no show rate combined with a long waitlist for appointments. Thank you for your understanding.
Medical/Surgical Appointments: To cancel a medical appointment, you may do so through your appointment text reminders or by calling our office at 970-247-1970. If you are calling outside of business hours, you are welcome to leave a message for our team. Patients who fail to show up for their scheduled appointment, without notice of cancellation, will be charged a $40 no show fee.
Cosmetic Appointments: To cancel a cosmetic appointment, you may do so through your appointment text reminders or by calling our office at 970-247-1970. Due to the nature of these appointments, cosmetic appointments require at least a 48 hour notice of cancellation. If you are calling outside of business hours, you are welcome to leave a message for our team. Patients who fail to show up for their scheduled appointment, without notice of cancellation, will be charged a $40 no show fee.
Payments
We accept checks, cash or credit cards. We also offer a flexible payment plan- Please ask for our Patient Liaison for details.
Once you receive your statement, payments may be made through the mail, online, calling the office at 970-247-1970 or in your patient portal.
Patient Portal
The patient portal can be used to communicate with your care team, view visit notes, and view or pay statements. If you need help logging in or navigating the portal, please contact our front desk at 970-247-1970.
